Nurses are responsible to provide exceptional patient care in an environment where quality, respect, caring and compassion are at the center of all they do. To provide professional nursing skills in assessing, planning, implementing, and evaluating the nursing care for assigned patients.

 

Promotes and restores patients' health by completing the nursing process; collaborating with physicians and multidisciplinary team members; providing physical and psychological support to patients, friends, and families; supervising assigned team members. You will report to the Head Nurse/ Assistant Head Nurse.

 

Main Responsibilities

  • Provides initial and ongoing assessment and interpretation of patients within the framework of holistic professional nursing practice, and in accordance with the principles of patient and family centered care.
  • Assesses and interprets patient ability (physical, social and psychological) to manage their health in order to transition their care or discharge.
  • Identifies and communicates patient or family needs to the interdisciplinary health care team and other providers of care as appropriate.
  • Provides ongoing monitoring of patient’s or family response to intervention; identifies deviations from expected outcomes and assesses change in patient status.
  • Develops, coordinates and updates an individualized plan of care, based on evidence, in collaboration with the patient and their family, the interdisciplinary health care team and other providers of care promoting continuity of care across the continuum.
  • Develops and coordinates a comprehensive discharge and education plan to meet the patient and their families’ needs, including health promotion, self-management, identifying and coordinating resources to promote continuity of care across the continuum.
  • Establishes and maintains a therapeutic relationship with patient or families.
  • Implements prescribed interventions (e.g. diagnostic tests, medications) and performs independent nursing interventions (e.g. patient and family education).
  • Ensures the implementation, coordination and ongoing evaluation of the patient care plan.
  • Provides counseling that assists the patient in developing, improving, or regaining skills, social and community supports, promoting health and recovering from illness and disability.
  • Consults with the interdisciplinary team and other care providers to discuss changes related to patient status, and modifies interventions.
  • Interprets medical information including diagnosis and treatment plans to patient and responds to patient questions and concerns.
  • Provides information and instruction to assist patients to understand health care practices and processes.
  • Utilizes educational material to support patients understanding of their health care, health promotion, health maintenance, and the enhancement of their individual well-being.
  • Liaises with other providers in the community to transition care.
  • Utilizes data obtained from the patient and their family and other health care providers to evaluate the plan of care including the patient’s responses to interventions and achievement of expected outcomes.
  • Supervises and evaluates care provided by direct care assistive and support staff.
  • Uses data and evidence based on best practice to evaluate nursing activities and interventions.
  • Documents complete accurate and relevant data for every patient according to Capital Health policies.
  • Provides a supportive environment for communications and decision making by patients and their families.
  • Advocates for and supports the principles of patient and family centered care, and the patient rights and well-being.
  • Assists patient and families, to become aware of issues relevant to their health care needs and provides information on resources and services within the health care system.
  • Recognizes trends in health care needs within patient groups served, and involves the patient in planning mutually expected outcomes where possible and appropriate.
  • Demonstrates knowledge and understanding of legislation dealing with patient’s rights and responsibilities in health care.
  • Demonstrates knowledge of the roles, responsibilities and rights of other nurses and other health care professionals.
  • Practices within professional, legal and ethical standards and is accountable for his or her own actions and decisions at all times.
  • Demonstrates professionalism in attitude, dress and behavior.
  • Contributes to positive team functioning and supporting one another.
  • Develops cultural competence in order to provide culturally competent care.
  • Role models the principles of continuous quality improvement by critical reflection of own practice, seeking meaningful feedback from peers, clients and manager. Accountable for reporting professional practice issues according to the JCIA standards.
  • Provides timely, comprehensive and complete nursing documentation by utilizing nursing process, which meets nursing quality standards.
  • Reports incidents on time.

 


Requirements

  • Matric/ Intermediate (Science) and  Graduate (Diploma/ BScN) from renowned Institute
  • PNC license (validity up to 6 months at least) and ACLS/ BLS
  • Safe medication Administration
  • Intravenous Cannulation
  • Conscious Sedation (as applicable)
  • Prefer experience of 2-3 years from a well establish organization as Registered Nurse.
  • Demonstrate evidence of recent and relevant experience in the specialized area and in an acute hospital setting. :
  • Communicate with the patients and their family members and listed to their concerns with patience.
  • Do long standing, walking, bending, kneeling and stooping all day.
  • To lift or move patients frequently or as per the instruction from physician. 
  • Nursing is a full-time position in a health care facility open 24 hours a day, 365 days a year. 
  • Rotating shifts as well as holiday and work during inclement weather will be required.

  • Planning and Organizing: Demonstrate flexible approach to job rotations, rostering, e.g. night duty, double shifts, theatre on call, attitude to work.
  • Self Control: able to maintain rationale and objective actions when faced with a difficult and stressful situation.
